Texas Notary Public Surety Bond Form (pdf)
Form to upload in the SOS Notary Portal for proof of bond required for the notary application submission. A notary public must secure a $10,000 surety bond. An authorized person for the surety company providing the bond must sign in the space provided.

Applicants who are only able to submit their Notary Public application to us on paper, must contact us at notary@sos.texas.gov or 512-463-5705 to request a paper application. Paper applications will take longer for the SOS to process than applications submitted in our online system.
