A reservation of entity name under the Texas Business Organizations Code (BOC) is effective until the 121st day after the date the application for reservation is accepted for filing unless the applicant files with the secretary of state a notice of withdrawal of the reservation (BOC § 5.104). This form should be used when an applicant wishes to withdraw an existing name reservation during the period of its effectiveness.
